Job Summary:

Received and analyzed incident reports, participate in an investigations if required and participate in improvement plan formulation for any identified error and risk. Coordinate and follow-up the hospital compliance with required accreditation standards such as Joint Commission International (JCI) and Central Board for Accreditation of Healthcare Institutions (CBAHI) standards as directed by the HOD.

Major Duties and Responsibilities:

Ensures through his/her actions, either directly or indirectly, that a superior quality of medical treatment, compassion, and understanding is given to every patient served at the CARE hospitals.

Received and analyzed all incident reports.

Investigate patient safety issues within the facility and participate in Root Cause Analyses of sentinel events.

Recommend and facilitate change within the organization to improve patient safety, based on identified risks through incident reports.

Design and implement educational presentations that facilitate the understanding and implementation of incident reporting system within the organization.

Prepares quarterly reports regarding incident reporting systems in the organization that includes comprehensive analysis detailing categories of errors, causes of errors and recommendations for improvement of current processes that could prevent future errors.

Ensures that all hospital services are provided in accordance with standards established through various accreditation standards (JCI & CBAHI and others) and ensures the continuous readiness for the accreditation process.

Develops the timeline for completion of all tasks associated with the accreditation process.

In coordination with other departments, assesses the hospital compliance with accreditation standards and ensures the development of the required strategies to enhance compliance.

Schedule and facilitate regular meetings with the established Accreditation Steering Committees by the hospital administration.

Maintains knowledge on current trends in specialty and makes recommendations for improvement.

Assist / facilitate QI Projects In coordination with other departments.

Maintains Confidentiality at all times.

Performs other applicable tasks and duties assigned within the realm of the employee’s knowledge, skills, and abilities.

Qualifications:

A minimum of:

Education:

BS Degree in Healthcare related field (Medicine, Nursing or paramedical specialties).

Training in Quality Improvement, preferable certified course in QI from recognize institute.

CPHQ is preferred.

Experience:

Minimum of three (3) years experience in a similar position in a healthcare facility, or two (2) years recent experience working in a Q.I. Department in acute care hospital.

Competencies & Skills:

Must be an advanced computer literate.

Excellent organizational skills and strong attention to detail.

Ability to organize and coordinate groups across the organization.

Ability to collect and organize data.

Ability to collaborate and communicate effectively both in writing and verbally.

Ability to prepare informative and accurate reports on program progress
