As a Home Care Nurse, you will be responsible for performing comprehensive health assessments, including physical, mental, and emotional evaluations of patients to determine their healthcare needs. Continuous assessment is essential for the effectiveness of care plans, and you will implement nursing care procedures as required to ensure the best possible care for patients.
Your responsibilities include developing individualized care plans based on patient assessments in collaboration with a multidisciplinary healthcare team. You will identify goals and necessary interventions to address patients’ health concerns and enhance their well-being.
Administer medications and treatments as per physician’s orders, ensuring adherence to dosages and proper administration techniques. Monitor patients for any side effects or drug interactions, and communicate any concerns to the appropriate healthcare provider.
Evaluate wounds and provide treatment, including the use of appropriate dressings, wound cleansing, and infection control measures. You will monitor the healing process and educate patients and their families on proper wound care techniques.
Provide education and support to patients and their families on various health topics, including medication adherence, disease prevention, and healthy lifestyle choices, empowering patients to actively participate in their healthcare decisions.
Maintain accurate patient records and documentation of assessments, care plans, interventions, and outcomes in accordance with hospital policies and procedures. Ensure compliance with legal and ethical standards related to patient privacy and confidentiality.
Demonstrate proficiency in basic first aid techniques and emergency protocols for handling acute situations and providing immediate care until additional assistance arrives.
Participate in continuous quality improvement initiatives within the home care department, contributing to the development and implementation of policies and best practices to enhance patient care outcomes.
Ensure proper maintenance of financial records required for the finance department.
Possess basic computer skills.
Adhere to specialized nursing procedures and collect necessary samples for laboratory testing.
Follow the guidance of the responsible nurse directly.
Report any medication errors, safety issues, or supply shortages to the responsible nurse or designated individual.
Engage in collaborative efforts with various specialists to achieve continuous improvement in the quality of patient care.
Assist in achieving efficiencies in unit costs.
