Position: Â FEMALE FILIPNO NURSE
Responsibilities:
Delivers direct nursing care as required.
Delivers care with appropriate age and cultural competence to specific patients according to individualized needs.
Using a structured process, incollaborationwith the patient and family, assesses, plans, delivers, evaluates, and documents an individualized patient assessment that reflects the patient goals and priorities. Provides consistent follow-up and modifies plans of care as needed.
Manages patient care by monitoring vital signs, performing physical assessments, and checking patients’ medications for effectiveness.
Performs appropriate treatments as ordered by the physician in an accurate and timely manner.
Recommends tools or devices that may improve thequality of lifefor the patient.
Evaluates the appropriateness, effectiveness, and efficiency of the interventions of care designed to meet patient needs and adapts as necessary incollaborationwith the patient, family, and the healthcare team.
Notifies the patient’s doctor and other healthcare team members of significant changes in a timely manner.
Collaborates with other healthcare professionals to provide patients with the best possible care.
Provideseducationto patients and families on proper home health care procedures and strategies.
Maintains strict confidentiality regarding knowledge of all patient information.
Maintains professional standards in relation to confidentiality, ethics, and procedures.
Responsible for establishing and maintaining healthy interpersonal relationships with all staff members.
Complies with the company’s policies and procedures.
